Plantar Fasciitis: A Guide for the Dancer
There are three steps you have to complete to become a professional dancer: learn to dance, learn to perform, and learn how to cope with injuries. -D. Gere
Plantar fasciitis is a common cause of heel pain on the walking (plantar) surface of the foot. It is most severe with the first steps in the morning and improves after walking. It worsens with prolonged standing or weight bearing. The plantar fascia connects the heel bone (calcaneus) to the bones at the base of the toes and stabilizes and distributes the force of impact sustained during walking. Inflammation of the plantar fascia may occur with tightness, overuse, and lack of support of the foot. Dancers are prone to the condition, as it is common in people with a high arched foot. Dancers often wear shoes with little support or padding and may spend countless hours standing and applying repeated forces to the feet. These repetitive actions cause micro tears in the tissue that produce inflammation and a searing pain on the medial (inside) walking surface of the heel. Many studies show that the condition is usually self-limited and the symptoms resolve in 12-18 months. For a dancer, this is an unacceptable period of inactivity and training, so it is critical to prevent plantar fasciitis through proper stretching and foot care and to treat it promptly when symptoms begin with a multi-faceted approach.
Prevention and Treatment
Despite many rigorous studies, no scientific evidence can demonstrate a definitive cure for plantar fasciitis. There are many common remedies that may provide relief if instituted at the onset of symptoms.
- Stretching – Prior to dancing, proper stretching of the calf and Achilles tendon should take place. A specific active stretch for the plantar fascia can be accomplished by sitting on a chair with the leg crossed at 90 degrees and the toes grasped and stretched gently upward until a stretching sensation is felt at the arch. Night splints that keep the foot in a gently flexed position may hasten recovery. Massage with a tennis ball while standing or sitting may provide relief.
- Strengthening intrinsic muscles of the foot – The intrinsic muscles of the foot are those that originate and attach within the foot itself and do not extend beyond the ankle. The general health of the plantar surface can be augmented by strengthening these small muscles within the foot. One method is to pick up marbles with the toes or grasp and release a towel with the toes.
- Foot support – When possible, the dancer should wear shoes that provide plenty of support and padding for the heel and arch. Well fitting athletic shoes are a good choice. Gel inserts to provide padding for the heel are helpful. About half of people who suffer from plantar fasciitis have heel spurs (a bony growth on the bottom of the heel). For these people, a small cut out in the heel pad may provide extra relief.
- Medicine – Many researchers believe that inflammatory changes in the plantar fascia account for the pain. Thus, the use of anti-inflammatory agents may provide relief. Ice may reduce inflammation, especially after exercise. Examples of over-the-counter agents are: aspirin, ibuprofen (Advil ® or Motrin ®), and naproxen (Naprosyn ® or Aleve ®). These belong to a broad category of drugs called non-steroidal anti-inflammatory (NSAIDs). There are countless other prescription NSAIDs. In addition to their anti-inflammatory properties, NSAIDs provide some analgesic (pain relieving) effects. Acetominophen (Tylenol ®) is an analgesic, but not an anti-inflammatory agent.
- Nutrition and vitamins – Proper nutrition is also essential for a healthy body, and the following supplements have a theoretical (but scientifically unproven) benefit in this condition. Vitamin C, Zinc, and glucosamine play a role in tissue repair, while Omega-3-polyunsaturated fatty acids (fish oil) may reduce the rate of production of factors responsible for causing inflammation.
While self-directed remedies may alleviate symptoms and promote healing in some people, there are other treatments that only health-care providers can perform. These include corticosteroid combined with local anesthetic injections directly into the plantar fascia. Repeated injections may weaken the fascia and pose an increased risk of rupture. Another therapy with theoretical benefits to stimulate healing is extracorporeal shock-wave therapy (ESWT), but scientific studies failed to demonstrate this to be uniformly beneficial.
For recalcitrant cases of plantar fasciitis, surgery may be a last resort. After failing all of the conservative therapy options, the surgeon can perform a release of the plantar fascia, excise any bone spurs, and remove scar tissue. Full recovery may take several months, and 1 in 4 patients may suffer persistent pain.
Heel pain has many causes ranging from neurologic, musculoskeletal, and systemic disorders. Consultation with your health care provider is the best way to diagnose and develop an effective treatment plan for your condition.

Teresa Johnson (of Santa Barbara Dance Center), has invited choreographers in their 40s, 50s, and 60s to join her in creating a production celebrating their longevity in the field of dance! The intent is to inspire people to abandon an "over the hill" mentality, and daringly adopt a "live at the top of the hill" attitude instead. This dynamic show includes contemporary, jazz, swing, Latin, dance theater, and belly dance. Each choreographer is using at least one over age 40 performer in their pieces, and most of the choreographers are performing as well.
