Happy new year! I hope you enjoy the first newsletter of 2009, which includes what I hope are helpful and informative submissions from our many members, as well as a wonderful introductory article by Wendy C. Coates, M.D., addressing dancers and their health care providers. Essentially "demystifying" the roles of practitioners we dancers seek out for care, this article is the first in a series by Dr. Coates that promises to answer questions many of us have had over the years when dealing with dance injuries and our health. Next issue (Summer 2009), Dr. Coates will address plantar fasciitis in dancers, a common ailment of so many dancers. I thank Dr. Coates for her enthusiasm and willingness in sharing her expertise with the readers of SCDC.

Dance and Medicine – A Partnership for Success

To be a dancer requires the mind, body, and soul to work in harmony. When one is disrupted, the imbalance produces a dissonance that permeates the daily life of the dancer. An injury may mean loss of income, inability to audition, and erosion of skills and fitness level. Ignoring the injury may lead to permanent damage. For dancers across all spectra, the inability to dance means a loss of self-expression and fulfillment. Medical professionals may not be familiar with the dancer’s need to maintain a high level of conditioning, and may simply advise the dancer to “take it easy” for several weeks or months to allow an overuse injury to heal. This is an impractical solution which dancers frequently ignore. It is incumbent upon dancers to be self-advocates when encountering the medical system so that they can be partners in healthcare decisions. This series of articles presents an approach to maintaining physical and emotional health by offering a scientific basis for dancers to understand their bodies and how to manage common issues that can affect well-being. The first article, “Who are your health care providers?” includes a brief description of the training and scope of practice of various specialists who impact the care of many dancers. Subsequent articles will focus on specific medical conditions that occur commonly in dancers. This series is not intended to replace the medical expertise of the dancer’s health care team, and dancers are encouraged to seek professional treatment for conditions that they may be experiencing.

 

Who Are Your Health Care Providers?

There are many members of the health care team. To maintain overall well-being, it is imperative to seek out qualified practitioners and to develop a working relationship with them to preserve professional or recreational dance goals. Readers are encouraged to learn more about what each can offer and to choose qualified primary and specialty care providers whose vision of health and recovery is aligned with the dancer’s.

  • Physician – A physician has a comprehensive education in the basic and clinical sciences and is often the director of the health care team. Physicians earn a bachelor’s degree and attend a four-year medical school program, where they earn the M.D. (Doctor of Medicine) or D.O. (Doctor of Osteopathy) degree. They focus on anatomy, physiology, and the pathologic basis of disease. Medical students rotate through all the specialties. After graduating from medical school, they continue their training as interns (first year) and residents, where they specialize in their desired area (e.g., primary care, surgery, obstetrics and gynecology). Physicians become licensed by passing a three step examination on basic science and the clinical practice of medicine. At the conclusion of their residency program, graduates are eligible to become “board certified.” This achievement represents mastery of one’s specialty, by passing a rigorous examination that involves in-depth knowledge and practical demonstration of skills. http://jama.ama-assn.org/cgi/reprint/284/9/1198.pdf
  • Chiropractor – A chiropractor specializes in disorders of the musculoskeletal system and commonly performs “chiropractic adjustments” or “spinal manipulation” to restore joint health. To become a Doctor of Chiropractic (D.C.) one first earns a bachelor’s degree and then enrolls in a specialized education program for four years of classroom and clinical training. Some chiropractors take additional training in a sub-specialty field to gain expertise with special populations. To practice chiropractic, one must pass the national board examination and obtain state licensure. http://www.amerchiro.org/
  • Podiatrist – Podiatrists focus on the health and diseases of the foot and ankle. A podiatrist earns a bachelor’s degree followed by the Doctor of Podiatric Medicine (D.P.M.) degree after attending a specialized school for four years. Typically, this is followed by two to four years of residency training and passing of a certifying examination. Some podiatrists are licensed to perform surgery on the foot and ankle.  http://www.apma.org/s_apma/index.asp
  • Nurse Practitioner – These medical professionals are licensed to practice independently, but frequently work in association with physicians to provide acute or ongoing care. Candidates for nurse practitioner must first become a registered nurse by earning a bachelor’s degree, an associate’s degree, or a nursing certificate. In most states, an advanced degree, such as a master’s in nursing (M.S.N.) is required. A specialty-specific educational program is then completed and certification and state licensure requirements must be met. http://www.aanp.org/AANPCMS2
  • Physician Assistant – A physician assistant (P.A.) works under the supervision of a physician to provide medical care in a variety of specialties and earns certification by passing a national examination. Most PA’s have a bachelor’s degree and significant health care experience prior to enrolling in PA school, which is typically a two year program. http://www.aapa.org/
  • Physical Therapist – Physical therapists diagnose and treat conditions that impair normal daily functional activities and work with patients to improve mobility or to restore function following an injury.  Their training begins with a bachelor’s degree, followed by specialized training that focuses on basic sciences and clinical sciences at an accredited school. They may earn a master’s (M.P.T.) or doctorate degree (D.P.T.) in physical therapy.  Subsequent training occurs in residency and fellowship programs and specialty certification is attained by testing. http://www.apta.org//AM/Template.cfm?Section=Home 
  • Massage Therapist – Therapeutic massage may be an effective adjunct to musculoskeletal health. Some therapists may be associated with a medical practitioner, while others may practice independently. Massage therapists attend a training program for a minimum of 500 hours and may become certified by a national board and must be licensed in their state. http://www.amtamassage.org/
  • Other – There are many other professionals who may play an integral part of the health care team for the dancer. Some examples are: personal trainers, kinesiologists, Pilates or yoga specialists, psychologists, and nutritionists.

The Trudl Zipper Dance Institute at the Colburn is pleased to inform you of a special course entitled “Introduction to: “Training the Dance Piano Accompanist”, taught by renowned dance accompanist Michael Roberts. This class is designed to instruct the musician on how to accompany both the Ballet Class, and the Modern Dance Class, and will culminate in observation of a Ballet Master Class by internationally acclaimed teacher Finis Jhung.  Accompanying a dance class is a unique skill that requires a strong sense of rhythm and phrasing as well as musical versatility. There is a great need for Dance Accompanists here in Los Angeles as well as across North America. These positions can bring a welcome extra income, as well as integrate the professional musician into the world of dance.
Mr. Roberts received his training playing for the Royal Ballet School in London, and was senior ballet accompanist for the Stanley Holden Dance Center for fourteen years. Most recently he was Music Coordinator for the CalArts Dance School for fifteen years. His many dance class recordings are considered the gold standard in the industry.

Distinguished Professor Yvonne Rainer, an avant-garde choreographer and filmmaker, presents a restaging of her signature work, The Mind is a Muscle Part 1, or Trio A. Jodie Gates presents an excerpted version of choreographer William Forsythe's neo-classical masterpiece, Artifact Suite, with 25 dancers.

Named after the Portuguese expression “saudade,” which is used to convey the longing, wistfulness and nostalgia inherent in the Portuguese blues music, Fado, this evening length work is an ode to the idea of “bittersweet.” Buoyed by mystical images of flight, the night sky and the deep southern bayou, and grounded in folklore, historical fact and personal experience, Saudade is a mosaic of character monologues told from a uniquely southern, African American perspective—the stories of oppressed people finding a reason to go on in a harsh everyday reality.
