Achilles Tendon Injuries: A Dancer’s Guide

The Achilles Heel – In Greek mythology, Thetis, the mother of Achilles, held him by the heel while she dipped him in the River Styx to make him invulnerable in every place the water touched him. Alas, his death came when his heel was pierced by an arrow in battle.
The Achilles tendon is the largest and strongest tendon in the human body. It connects the calf muscles (gastrocnemius, soleus, and plantaris) to the heel bone (calcaneus). When the calf muscles contract, they pull on the Achilles tendon causing it to shorten and the foot to point (plantar flexion). Contraction of the Achilles tendon is necessary for walking, running and to rise to demi or full pointe and to jump. Despite its size and strength, the Achilles tendon is a common location of overuse injury that is potentially incapacitating in professional and recreational athletes, particularly dancers. Risk factors for developing Achilles tendinitis are intrinsic (biomechanical abnormalities that are unique to a particular athlete) or extrinsic (preventable and treatable). Common intrinsic factors include anatomic instability (arches are too high or too flat), muscle imbalance (opposing muscle groups have different levels of strength) or lack of flexibility. Extrinsic factors can usually be corrected with diligent effort and include causes like improper technique, hard floors, footwear, drastic changes in intensity or frequency of exercise. Repetitive stresses to the tendon cause micro trauma and tissue damage. This manifests itself as pain and tightness along the tendon. In the early stages of injury, there is localized pain during or following activity. As the condition progresses, pain develops earlier during activity or at rest. Swelling or a hard knot of tissue (nodule) may be a sign of tendon degeneration that may not hurt, but should be evaluated by a medical professional.
Prevention
Several interventions may reduce the risk of developing new or recurrent Achilles tendinopathy.
1. Warm up - Always perform a proper warm up prior to activity, including stretching exercises specifically targeting the calf muscles and the Achilles tendon. Heel lifts with a straight knee stretch the gastrocnemius muscle, while heel lifts with bent knees stretch the soleus muscle.
2. Avoid training errors - Increase training in gradual increments. Avoid over training and training with excessive pain. Sudden increase in intensity is a risk factor for injury.
3. Proper footwear – Dancers should wear well-fitting shoes. Constricting ribbons or elastics that put pressure on the Achilles tendon may inhibit its range of motion and lead to tendinitis.
4. Ensure proper dance mechanics – Proper technique is at the core of injury prevention. The use of plié and replacing the heel completely on the floor when landing any jump avoids Achilles tendonopathy.
5. Medications that affect the Achilles tendon – Some medications have been associated with Achilles tendinitis and tendon rupture, particularly antibiotics of the fluoroquinolone group (Cipro/ciprofloxacin, Ofloxin, Levofloxacin/Levaquin). If one of these antibiotics is necessary to treat a medical condition, pay special attention to Achilles tendon health and report any pain immediately to your health care provider. In some cases, a different antibiotic may be effective.
Treatment
The goal of therapy is to relieve symptoms and enable return to activity. Here are some helpful treatment strategies:
1. Rest - If symptoms are mild, avoid exercises that stress the Achilles tendon, such as jumping. For severe symptoms, an extended period of rest may be necessary. Cross-training with low-impact exercises such as swimming can help maintain fitness during are recovery.
2. Ice - The application of ice to the Achilles tendon when symptomatic can help reduce inflammation and pain, especially after exercise.
3. Medication - Taking a short course (7 to 10 days) of nonsteroidal anti-inflammatory drugs (NSAIDs) can help to reduce pain and inflammation, and enable return to activity sooner. There is no scientific evidence supporting the use of topical NSAIDs for pain or inflammation relief. However, some dancers report relief of symptoms with topical agents such as Tiger Balm, Icy Hot, or other over-the-counter remedies.
4. Foot Support - In some cases, orthoses, which are devices to help support the muscle and relieve stress on the tendon such as a heel pad or arch support insert, can be useful in relieving symptoms associated with Achilles tendonitis. Sometimes these can be used while dancing, but can definitely be used in street shoes to provide support and recovery while not engaged in activity. A bandage or wrap specifically designed to restrict motion of the tendon can also be used, especially while training is necessary after injury.
5. Eccentric exercise – These exercises reduce the time needed to return to activity. Eccentric exercise is the application of a load or weight during the lengthening of a muscle. (This is in contrast to concentric exercises, which involve contracting a muscle against a load, e.g., relevé.) Stand with the heel of the affected foot beyond the edge of the step or platform with the foot plantar flexed (picture A). Slowly lower the heel, bringing the foot into dorsiflexion (picture B). As with stretching, performing these exercises with the knee straight will target the gastrocnemius muscle and with the knee bent will target the soleus muscle (picture C). To avoid inadvertent concentric exercise, raise the affected foot back to the plantar flexed starting position using the unaffected foot, and hands using the barre. A typical regimen is done twice a day for 12 to 24 weeks, performing 3 sets of 15 repetitions with straight knees, then 3 sets of 15 repetitions with bent knees.

6. Steroid Injections – Steroid injections are NOT recommended in the treatment of Achilles tendon injury because they may weaken the tendon and lead to a risk of tendon rupture. Furthermore, there is no medical evidence demonstrating any benefit.
7. Platelet-rich plasma injections – some practitioners endorse the injection of platelet-rich plasma to speed up healing of Achilles tendon injuries. They theorize that it improves the blood flow in the area of the tendon (which normally does not have a good blood supply). Scientific trials have not shown a clear benefit for this therapy, but some dancers report improvement in their symptoms if performed soon after injury.
8. Finally, for dancers with Achilles pain that persists despite the above treatment and prevention modalities, surgery is a treatment option of last resort. Recovery is slow and involves months of rehabilitation. Surgery does not guarantee a return to dancing at a pre-injury level of activity.
If you are experiencing tendon pain, consult your health care provider early. It is the best way to diagnose and develop an effective treatment plan for your condition.

Mr. Wagner has been teaching Master Classes in the Coachella Valley area for the past few years and has decided to develop a new school, The School of Paul Wagner’s BalletsUSA. With a name reminiscent of the early Jerome Robbins company of many years ago and also the founding of a support arm like Balanchine and Kirstein’s linkage to Ballet Society the two will try to map out a bright new expanded future for dance excellence in the Palm Springs Coachella Valley area.

• Double The Learning Drive (Funding Deadline: March 1, 2010)
C. Derrick Jones, Co-Artistic Director, was awarded the $10,000 Artist-In-Residence grant from the LA Department of Cultural Affairs, to design and implement our 10-week dance-theater workshops about partnership and trust. We are launching a fundraising drive to DOUBLE this amount so that we can develop and publish a book of our techniques with a companion DVD, for personal and educational uses.
• Creative Seeds Challenge (Funding Deadline: May 1, 2010)
Nehara Kalev, Co-Artistic Director, was awarded the coveted Djerassi Resident Artists Grant, which provides one month of uninterrupted creative development time at an international artists’ retreat in central California. It includes room, board, and rehearsal studio, but does not provide a stipend to replace lost income. We need $3,000, which will allow both of us to take full advantage of this artistic residency, where we will investigate new dance-theater approaches for the Ford Amphitheatre premiere of IRON.
IRON: Year Six Performance Fund (Funding Deadline: July 1, 2010)
We have been invited by The LA County Arts Commission to premiere IRON: Year Six on FRIDAY AUGUST 13, 2010. A wonderful opportunity for us, but we need $47,000 to support set design, aerial rigging, aerial set fabrication, rehearsal, materials, technical assistance, publicity, collaborator stipends, lighting, sound, video, costumes, and production coordination.

Joni Mitchell's "The Fiddle and The Drum" is a ground-breaking collaboration between legendary artist Joni Mitchell and Alberta Ballet Artistic Director Jean Grand-Maître. This semi-abstract narrative ballet is danced in neo-classical style and performed to Mitchell's compelling music and her latest artwork, which is projected on large canvas screens behind the dancers throughout the ballet.

Based on the science of quantum physics, the work examines how two or more objects are linked together even though they may be spatially separated. This site specific event begins in the Martin Luther King Auditorium with a live performance with dancers interacting with projections on a video screen and a short film presentation. The audience then moves to the courtyard, where the dance performance will continue. Admission is free.

Adapting the evening-length work, Quantum Entanglement, to a proscenium stage, DS&D explores the concept of entanglement from both a scientific and human perspective, mixing up the subatomic movement of particles with the emotional ramifications of being entwined with people and situations.
